Fail safe means that any system failure will create the best
unconditionally "safe" situation that is possible under the
circumstances.  This includes conditions which may not be immediately
evident to the driver.

Even a casual peerusal of the published accounts of train accidents
around the world is adequate to indicate that there are many factors
beyond the immediate knowledge of the driver which contribute to
accidents.
