The govt makes up the difference for concession fares. In my
view, better to have a concession fare passenger with the
difference made up than an empty seat.

A bulk billing doctor does not treat patients differently if
they choose to pay fees upfront then claim back through Medicare
compared to if they use the doctor's bulk billing service.
