I think it is something like this ..... Sticking brakes (or brakes not
properly released) can prevent an axle from rotating, so that the wheels
skids along the track, picking up a very fine shim of steel off the head
of the rail until it builds up into a lump of scale stuck to the tread
of the wheel. If brakes then free up, the rotating wheel will lift off
the rail each time it runs over the scale build up. If the scale is
thick enough it can derail the vehicle.
