The red signal, "R" below, of SN109 is offset to the left like so:-

R G

And immediately to the left of the "R" was a square signal panel of some
sort.

The driver of the SPAD train had only been driving for about 25 days after
just coming out of training. The SN109 being of a strange configuration
probably fooled the inexperienced driver. There have been complaints about
this signal on many occasions in the past apparently, the last as early as 6
weeks ago.
