Quite simply the level crossing didn't detect the train coming, or the
level crossing wasn't working/had no power.

There is no protection of a level crossing expect where stopping/express
selection is provided, and this is to stop a express train selected as 
stopping running through a level crossing not operating.  There is a very
good book in the State Library (Vic) about level crossing protection in
Victoria explains everything and why it is done that way (not that many
people would aggree with it, but at least it is explained)
