This one crops up regularly. There was a long thread on it in
misc.transport.rail.europe last year - try dejanews. I can't find my
collated answers.
AFAIK the winner was Postlingberg, a mountain tram starting in Linz,
Austria. Hakone tozan mountain tram in Japan, 120 km south of Tokyo, is
steep but beaten (AFAIK 1 in 12).
