Reading this reminded me of when I spoke to my safeworking friend about this
thread on trip signalling:
There is an inherent fault in trip signalling - the driver. (Before you
flame me for hanging it on drivers - I'm not - and I don't want pornographic
images in the e-mail thank you either 8^) <teacher mode on> I provide my
return address on a trust basis - that I trust people will use it properly.

Anyway, He told me the story of a train accident near Caulfield,
Melbourne (not sure how far back) where the train was tripped but the driver
kept resetting the brake and kept going where he collided with the train in
front - glad to say he didn't hit it too fast. Perhaps someone with more
experience in such areas can comment.
