Not sure about "official" findings and publication.

But a cornfield meet in Train Order territory usually
means one of two things.  
An incorrect train order was issued, has happened but
very rare.
Or somebody working long hours without a decent rest
and all the little worries and human characteristics we
all have, forgot what was on it.?

There is something that can help and ARTC are using the
old CR/ANR/AN procedure which was not always used on
the bottom of the Train Order;

"REPORT THROUGH ALL SIDINGS"
