Interesting.  There was an article in a recent TRAINS magazine about a
head-on which occurred in exactly the same way.  The man on the
ground, off the freight train waiting in the siding, for reasons
unexplained, changed the turnout as the passenger train approached.

Human error - it is the reason for most accidents.
