First of all the crew would be considered guilty until proven innocent. The
crew would be immeadiatly stood down, the signalman or train controller however
would probably keep working.

After a while there would be an internal investigation by the railway, the
point of which would be to confirm the crews guilt. If eveidence came up that
could be verified by at least 6 independent sources the crew may get off. Any
signalling staff would automatically be absolved because their signalling
systems are 100% fail safe.
