FreightCorp (ex SRA, ex PTC of NSW, ex NSWGR) taking a 'no blame' focus!
Unbelievable. Some 20 years ago when investigating a safeworking breach for
the then PTC of NSW I suggested in my report that the guard and driver were
not entirely at fault due to certain circumstances surrounding it that were
not under their control and I proposed a means by which this breach could be
eliminated. I was told that you should never offer any excuses for people
under investigation.

The typical lawyer's approach.
