Pretty vague!!
By brake failure, what do you mean?
1/ BP blockage?
2/ Defective brake valve, not allowing air to exit to atmosphere?
3/ Excessive piston travels on cars?
4/ Sudden dynamic failure
5/ Defective signals
It might be a good idea to resist the tempation to post until you have some
facts instead of speculation/rumour/opinions.
