1/2/97: LIVERPOOL: UNIDENTIFIED PERSON STRUCK AND FATALLY INJURED
BY 26B-E RUN THE 1049 HOURS CAMPBELLTOWN TO BLACKTOWN SERVICE.
DRIVER XXXXXXX OF LIVERPOOL DEPOT AND GUARD XXXXXXX OF LIVERPOOL
DEPOT IN CHARGE OF 26B-E RUN AT TIME OF INCIDENT.

XXXXXXX STATES: I am employed by the State Rail Authority of New
South Wales as a Senior Inspector. On Saturday the 1st of February
1997 I was rostered to do weekend supervision signing on at 0600
hours. At 1120 hours I was informed by Operations Control that
there had been a fatality at Liverpool involving 26B-E run. I then
travelled passenger to Liverpool on 51-A run departing Central at
1135 hours and arriving at Liverpool at 1230 hours. On arrival I
went to the Station Masters office and informed him of who I was and
he advised me that 26B-E run was already in the sidings and that
Service Control Superintendent XXXXXXX was down the sidings with the
investigating Police Officers. I then proceeded down to Up sidings
where I met Service Control Superintendent XXXXXXX and the Police
Officers where I was informed of the situation. 26B-E run consisted
of the set T14 with the carriages in direction of travel being D6251,
N5251, N5252 and D6252. I was informed the the person had jumped
down off number 2 platform as 26B-E was departing number 1 platform
and threw himself under the third position carriage N5252 and was
run over by the number 3 wheel. Driver XXXXXXX had just been
relieved by Driver XXXXXXX and heard a cry from the other side of
the train and indicated to the Guard to stop the train which was
done. The point of impact was found to be 35.622 Km on the Up Main
line and the point of recovery to be 35.615 Km in the six foot
between the Up Main and Down Main line. Both the Driver and Guard
were breath tested by Constable XXXXXXX of Liverpool Police both
returning a negative result. The body was lifted clear at 1155
hours and conveyed to the hospital. The Down Main line was
re-opened at 1200 hours and the Up Main line was re-opened at 1208
hours after 26B-E was shunted into the Up Storage siding.
