"They included installing an automatic train braking device to stop a train
going past a red signal. Already 112 of these had been put in place."

They have? When? Last night?

"He said he was "disturbed" that there were a number of signals drivers
repeatedly passed when red, and these were being fixed."

I didn't know they were broken.

"Thirteen catchpoints - the areas where a train automatically derails when it
runs a red signal - had been repositioned because they were dangerous. At some
points it was found that if a derailment occurred, the train could fall down
an embankment."

Whoever authorised them in in the first place should be sacked and sued.
