I don't think he is referring to failed signals. I think he is
refferring to the numerous incidents of drivers passing red signals then
having a train derail due to catch points.

Sometimes it can be problems with signals. Once the signals showed
double green on approach to the Dives from the south, the next signal
was yellow over red. We were quite promptly stopped by the train-stop
(and I got very intimate with the piece of glass in front of me).

In 1999, there were no less than three occassions on the same signal
where the signals showed caution correctly and the driver still tripped
the system. So while some incidents may be caused by signal failure I
would argue many more problems are caused by misjudgements by drivers.
