ATSB - Incident Report

In an 11/11/99 press release, the Department of Transport and Regional
Services reported that there had been a lack of effective communication
between a ship's pilot and the officer of the watch prior to the grounding
of a cargo ship on Heath Reef.

The ship's pilot had been asleep for nearly an hour before the grounding!

I hope that the newly-formed ATSB is capable of a more incisive finding in
future investigations, be they air, sea, road or rail related!
