Concord West etc

In view of the outcome of the Concord West derailment inquiry, and the
probable outcome re Hornsby, surely some sort  of AWS (Automatic Warning
System) MUST now be on the agenda? Or do we need another Granville-scale
incident to convince the powers-that-be to ignore the cries of the
bean-counters who argue that insurance premiums and compensation payouts are
cheaper than installing hardware?

I noticed in Japan, that all train drivers seem to have some little ritual
they perform whenever they pass a signal - it consists of raising the index
finger of the right hand as if to say "yes, seen that one". Is there/could
there be any such equivalent here?
