A few bits of info...

There was a signal failure.  Two signals failed to stop, the signal in
the rear of that was at caution.  The signalling system failed safe.

WL-2 stopped at the first signal for a while, and rang Penrith who
advised him that the signal was a failure and to pass it in accordance
with the regulations (under extreme caution, prepared to stop short of
any obstruction).  WL-2 then proceeded to the next signal at stop.  The
signal phone on the next signal was defective, so after waiting one
minute, started to pass it in accordance with the regulations for an
automatic signal.  As the brakes were coming off, W534, which had passed
the signal in the rear at stop, collided with the rear of the train at a
speed unlikely to be considered extreme caution.  W534 had been
authorised to pass the signal by Penrith box, but did not travel under
extreme caution.  The impact caused WL-2 to break in half by breaking an
auto coupler in approximately the centre of the train.  The front half
of WL-2 proceeded a further 20 metres before it stopped.  The rear half
moved approximately 5 metres.  The car carrier on the rear of WL-2
sliced through the drivers cab, crew compartment, vestibule and centre
seating section of the lead car of W534.
