If the signals were working 100% the driver should take the blame. However, the
wisdom of low speed turnouts on high speed lines has to be questioned. In
England there is some formula for installing turnouts on high speed lines. I
don't know the figures but its something like: 125MPH mainline cannot have a
turnout slower than 50MPH. If there is a low speed diverge, like at a junction,
you get more than the usual warning of it. AWS helps too!
