The correct procedure for Melbournes system is:

If the tram/bus/train station is manned, paper recieves a paper ticket
explaining the problem for the day. Passenger notes Which bus/station
etc kept his ticket and contacts the 1800 #. Technician arrives to fix
validator, upon finding jammed ticket it is handed into head office and
a replacement ticket is issued to customer who lost the ticket. (usually
with a few days extra travel on it because of inconvenience).
