well you do have the silly people looking for the toilets who dont/cant
read english and dont have the common sense to say "hmm, whatever is
behind this door must be very small" but i am of the beleif this this
person must be like the guy from "pure luck" to "accidently" fall out
the door, like it was said before, it is simply too hard to do, all that
is needed to make the doors almost foolproof (nothing is completely
foolproof) is a small solenoid or strong magnetic latch on those little
flaps over the handles, surely there is a spare analogue cable between
the two loco's , then all it would require is a PA to check all the
doors BEFORE they depart the station
