The first news at Delec, Enfield was that the train had "run out of
brakes". The crane crew said the train had passed 3 sticks at stop and the
train brake was in the emergency position.

A friend at NR told me that either the day before or after the crash,
another train up there had been found with the taps up behind the loco.
This would explain having no brakes. Most unit trains have no air leakage
so it wouldn't be noticed.
